Please complete this form or call us to order your pet’s repeat prescription.
(Please allow 24 hours for us to prepare your items and if submitting this online form please wait for your email confirmation before coming in to the practice to collect).

Please allow more time for us to process large orders (50+ tablets or multiple items.) We will advise you of a collection time in your confirmation email or over the phone.

Note on collection times: During surgery times collection of drugs might take longer than normal, save time by coming out of surgery hours.
The quietest time for collection is between 8:30am – 12:30pm and 2:30pm – 4:30pm

Did you know that…?
You can collect worming tablets, flea treatments or repeat prescriptions anytime throughout the day.

Please note all repeat prescriptions are for a maximum of a months supply, unless otherwise agreed by a vet.

I understand that Fishguard Vets will respond to this email within 12 hours of receipt (Working office hours only).

I will wait for this email to confirm a prescription collection time before coming in to the Practice. Please allow 24 hours for us to prepare repeat prescriptions.
